Sometimes an old injury seems irrelevant, or perhaps simply slipped your mind. Maybe there was a rush to arrive on time, or you forgot your list of medications. Unintentional omissions in your health history are bound to happen!

For you and/or your family to receive the very best possible care, it can take time to ‘get it right’ and bridge gaps in your health history.

Your medical and chiropractic care (current, past and future) should include a complete list of all medications AND nutritional supplements when assessing your prognosis and determining an effective care plan. We’ve also provided a WebMD link to help understand how we can do as much as possible for you.

It has both direct and indirect control over every part of you- not just pain- and it’s critical that we have a “head to toe” understanding about all of your bodily systems to get the most out of your care.

Your immediate concerns, family history and a review of systems– a detailed look at your full-body state of functioning- give both of us an advantage to do the most in the shortest amount of time.

Please make or keep a copy for your records and take notes of anything you may have forgotten before your actual appointment so we can be as up to date as possible.
